Grading of joint osteoarthritis by Ahlbäck uses plain postero-anterior weight-bearing radiographs to assess the extent of degenerative disease as joint space narrowing and bone attrition. The Ahlbäck criteria (focusing on joint space narrowing) and the Kell- gren & Lawrence classification for knee OA were used for diagnosing tibiofemoral OA. RESULTS A questionnaire was sent to 2000 randomly selected people aged 35–54 years. The response rate was 92.6%. Fifteen per cent of these people reported chronic knee pain. Ninety-six knee radiographs of patients with clinical signs of arthritis were evaluated and classified into five grades of Ahlbäck classification by three observers with different experience.

Although the surgical indication for total knee arthroplasty (TKA) should be based on pain, clinical status, and the deterioration of quality of life, the radiographic study is the most commonly used criterion for preoperative evaluation. Thirty-seven knees had grade-1 Ahlback osteoarthritis in the lateral compartment on the preoperative radiographs, and one had grade-2 Ahlback osteoarthritis. The preoperative skyline view of the patellofemoral joint showed no bone loss with eburnation and longitudinal grooving in all the cases. In severe osteoarthritis patients (Ahlback grade 3, 4, 5), this treatment was of less benefit if those patients were young, active, and expected independent ambulation.
